At your first session, we would encourage our patients to download the new patient form available below, beforehand and to print and fill it in at your leisure.

Sessions can last between 30-45 minutes for the initial consultation so please allow for this. If you are unable to print and fill in the forms, please ensure you bring your reading glasses (if you need them) to fill them up at the clinic.

*Please note that we require a minimum of 24 hours notice should you need to cancel your appointments.

Missed appointments will be charged a £10 administration fee.*
